A Checklist for Your Health: Setting Goals

You can make lifestyle changes!  This simple checklist will help.

  1. Begin by making a commitment to change for yourself, and not for someone else
  2. Set realistic goals
  3. Break large, long-term goals into smaller, short-term goals
  4. Focus on the behavior you want to change instead of  on the outcome of your goal (such as weight loss)
  5. Put your plan in writing and post it where it can serve as frequent reminder
  6. Ask for help from others and be specific in your request
  7. Keep a written log of your progress
  8. Reward yourself when you reach an identified goal
  9. Remember, change takes time -- be patient with yourself
